Practice Members and providers have rights and responsibilities to one another to ensure that the best health care services are provided.
Practice Members and providers have the right and responsibility to treat one another respectfully.
Practice Members have the right to confidentiality when receiving care from providers.
Practice Members have the responsibility to supply accurate and complete medical history information to the provider.
Practice Members have the right to know that a record will be kept of the health care services provided to them. They may ask to view, obtain a copy, or amend or correct that record.
Providers will not disclose a client’s record to others unless directed to do so, in writing, by the client, or unless the law authorises or compels them to do so.
Providers have the responsibility to inform clients about their health condition and include the client in decisions affecting their care.
Practice Members have the right to bring questions, concerns, complaints or compliments about any aspect of one’s care or service to the individual provider, their health plan or provider network.
Treating Children
Children are always welcome, however young they are. We believe that prevention is better than cure and aim to prevent problems before they occur. Please note that a parent or guardian must be in attendance while we treat any young practice member under the age of 16.
We work as a multi disciplinary team to provide the best health care for every client. This may involve the option of cross referral within the clinic after discussion with both practice member and practitioner.
